Chronic Care Management (CCM) is a new program available to Medicare patients living with more than one chronic condition, such as arthritis, diabetes, depression, or high blood pressure.

Care Coordination

Southern ENT Associates offers a service that can help you navigate these challenges. Our CareHarmony care coordination service is designed to help you take care of you! You will benefit by receiving monthly telephone assistance in between your regular appointments. This includes a dedicated Care Coordinator to guide you towards better health.

Chronic Care Management extends your care outside of the four walls of your physician's practice to ensure that you maintain the best possible health. Each month your Care Coordinator will call you to:

  • Assist with scheduling appointments, lab tests or other tests
  • Explain how and when to take your medications
  • Coordinate any home health or medical equipment needs
  • Connect you with health education resources, services and programs
  • Identify available community resources
  • Coordinate follow-up care after leaving the hospital
  • Access other support services you need to stay well

There is also a 24/7 Care Coordinator Phone Line to help answer any questions you may have day or night outside of your calls with your Care Coordinator.

Cost and Coverage

This service is covered by Medicare and is subject to your annual deductible and coinsurance (usually 20%). If you have a secondary or supplemental insurance plan, your coinsurance may be covered. It is best to review exactly what your insurance plan covers.

While you can only sign up for Chronic Care Management services with one of your CareHarmony care coordinators, you are able to opt out any time—there are no strings attached.

What is CareHarmony and how will it benefit my care?

CareHarmony is a service that helps manage and support patients between visits. A dedicated care coordinator works with your provider to build a personalized care plan and coordinate care across all your other providers (e.g. primary care, cardiologists, endocrinologists, etc.), help you with your medications including refills, appointments and reduced risk of missed follow up visits, referrals, the 24/7 availability to answer questions and quicker help when problems come up both during and outside office hours, better tracking of chronic ENT-related conditions (like reflux, sinus disease, sleep issues), and support with other health issues that impact your ENT care.

Is CareHarmony a legitimate service, or is it a scam?

CareHarmony is a legitimate, trusted partner working directly with our medical team. We understand there are many scams today, but we want to reassure you that this service is run by professional nurses dedicated to coordinating your health. They will never ask for financial information or your Social Security Number.

What is the cost for CareHarmony?

The cost for the service is set by Medicare, which covers about 80% of the service fee. You are responsible for the remaining 20% copay, which typically amounts to only $10 to $11 per month. If you have secondary insurance (a supplemental plan), it often covers this copay for you. You can always consult your insurance provider for more information on CareHarmony coverage.

I didn't speak with a CareHarmony nurse last month, will I still be billed?

Yes, you might still see a charge. When you enroll, a nurse is assigned to you to provide dedicated, proactive care. Their work involves continuous review of your medical records for prescription changes, specialist notes, or hospital visits. They manage and coordinate your care behind the scenes, even when you don't require an immediate phone consultation.

I don't recall signing up for CareHarmony, or I thought it was free.

Enrollment always requires your explicit verbal consent given directly to the CareHarmony team over the phone. If you believe there has been a mistake, please contact your CareHarmony coordinator.

Am I locked into a long-term contract with CareHarmony?

The program is 100% optional and requires no long-term commitment. If you decide the service does not meet your needs, simply let your Care Coordinator know. Your enrollment will be canceled at the end of that calendar month.

What is a chronic condition?

A chronic condition is a long-term health issue that requires consistent management and treatment. Without proper care, it can affect independence and overall well-being. Common examples include diabetes, arthritis, asthma, heart disease, and high blood pressure.

What is care coordination?

Care coordination involves organizing patient care activities and sharing essential health information among healthcare providers to improve patient outcomes. Medicare's Chronic Care Management program offers this service, providing extra support between doctor visits for patients with two or more chronic conditions.

Who is eligible?

Medicare beneficiaries with two or more chronic conditions expected to last at least 12 months or until end of life qualify for the Chronic Care Management program. Contact your primary care provider to confirm your eligibility.

Do I have to be a Medicare member?

Yes, this program is currently only available to Medicare patients.

Most Medicare patients have secondary insurance that may cover CCM program copays. If not, you might have a small out-of-pocket cost (usually 20%) plus any applicable deductible. It’s best to review your specific insurance coverage for details.

Is my information private and secure?

Yes. Your medical information is protected under the same strict HIPAA privacy and security regulations that apply within your doctor’s office, ensuring confidentiality and data protection.
